LAMP (Life-Affirming Medical Proxy)
Due to numerous calls from people seeking our help to obtain a medical power of attorney (a.k.a. a durable power of attorney for health care) in emergency situations, the Healthcare Advocacy and Leadership Organization (HALO) has developed the LAMP document. It can be readily downloaded, signed, and witnessed when a person, who has not previously completed a life-affirming medical power of attorney, is about to undergo emergency surgery, has been seriously injured, etc.
Organ Donation Refusal Addendum to Medical Power of Attorney (MPA)
With this addendum to your MPA, you can elect to:
- Decline to be an organ donor.
- Indicate you do not want any tests performed for the assessment of “brain death.
- Document you want your medical proxy (a.k.a. healthcare agent, surrogate, attorney in fact) to see to the disposition of your mortal remains.
- Direct that all explorations of “medical futility,” treatment limitations including do-not-resuscitate orders, or bioethical assessment be first brought to your proxy’s attention and reviewed and approved or rejected by your proxy before entry into your medical record

I Refuse To Be An Organ Donor Wallet Card
Every state maintains an Organ Donor Registry listing people who have agreed to be organ donors, either by driver’s license or by signing an organ donor card. NO STATE (according to HALO’s research) has a registry for those who do not want to be organ donors. Therefore, it’s up to you to protect yourself. Sign and carry this wallet card with you.
